Mission Statement
The Institute for Community Health and Wellness improves health, wellness and equity by addressing social determinants of health (e.g., housing, food, education, transportation). With an integrated, community-informed approach, we connect public health and clinical care through health education, disease prevention, immunizations, community outreach and surveillance. Guided by local needs and community partnerships, we develop tailored programs and wraparound support for clinical, behavioral and social needs, empowering individuals and families to thrive in healthier communities.
